Iron KurtainTake 1000 points of damage in a single life.
This achievement is one of the first ones you should get as a Heavy in normal play. If you are working with a Medic, go on the offensive and push towards the enemy, retreating when you are faced with overwhelming force. If you manage to stay alive long enough, you will get it pretty easily. If you don't have a friendly Medic to heal you in the front lines, play defense and heal up at your cabinet whenever you take 150-200 points of damage. You'll get it pretty easily as long as you play it safe.
To grind this one, have a Medic (or Medics) heal you and get a few enemy players, preferably Medics as well, to shoot you with their weakest weapon (in this case, the Syringe gun or Blutsauger). If you get 4 enemy players, you can also get Quadruple Bypass for your healing Medic. Just make sure that you let the enemy team know when to stop shooting if you are near death.
Also, in order for this achievement to register, you must die once reaching 1000 damage taken or higher.
Tip: Remember that you have exactly 300 health, and that you overheal to 450 health. Use it to keep track of how much damage you have taken.
Party LoyaltyKill 50 enemies within 3 seconds of them attacking your Medic.
This one takes a combination of luck, skill, and awareness of your attached Medic. Basically, if you see your medic begin to take damage on the little "being healed by" bar at the bottom, quickly turn around and find who is damaging your friend. Them kill them. Do this 50 times (not in a single life, that would be a nightmare) and you got it! It should be something you are doing anyways.
To grind this one, get a bunch of Scouts on the enemy team. Have them hit your Medic one at a time with a baseball bat, and then unload on them afterwards. It's also a great way to get Permanent Revolution, as long as you don't spin down between Scout attacks.
Tip: This one can't be done on friendly fire servers. Don't even try it. You'll kill your own medic way too fast.
Division of LaborKill 20 enemies with a Medic assisting you, where neither of you die.
This achievement is really just the same achievement as Big Pharma mentioned in the first part of the Medic achievement guide. As a Heavy, though, slightly different strategy applies. To do this normally, you probably want to play defense on a map like 2fort or Goldrush. Make sure you have a Medic consistently healing you as you mow down enemy players. Much like Big Pharma, this achievement is incredibly hard to pull off in normal gameplay.
To grind this achievement, have the entire opposing team play Scouts and make them run at you and your Medic combo. It's a great way to get Permanent Revolution, Red Oktoberfest, Don't Touch Sandvich, and Heavy Industry.
Tip: Just like Big Pharma, only the Medic getting the assist points counts.
Red OktoberfestEarn a domination for a Medic who's healing you.
This achievement should be fairly easy to get, at least if you have a Medic staying with you most of the time. Just kills lots of players. If you and the Medic stay alive long enough, chances are you'll both get a domination on somebody. It's an achievement you can get pretty easily in normal play, just play normally and be a good Heavy with a good Medic.
To grind this achievement, have Scouts run to you and blast them away. Kill the same Scout three times, and you'll earn both you and your Medic a domination. It can be done with any class, but Scouts die the fastest. Extremely simple and easy to grind.
Tip: It doesn't have to be a Domination from scratch. The Medic can kill a few times himself. As long as the domination is got through the assist, the achievement will count.
Crime and PunishmentKill 10 enemies carrying your intelligence.
This one is more of a time thing than anything else. As a Heavy, play a defensive game. Obviously, it's easiest on 2fort, given the layout of the map. Good places to hang around when you know the enemy is still trying to escape with your intel is the exits for both the straight and the spiral stairs. Another good place is in the courtyard, as most players go up the straight stairs to try and escape.
To farm this one, get a bunch of enemies in your intel room and have an engineer set up a dispenser next to you. Have them pick up the intel individually, and then unload on them. The dispenser is to both keep your ammo up as well as work on the Factory Worker achievement, which requires kills while next to a dispenser.
Tip: The enemy must be holding the intel. If they drop it right before death, or you kill them while they are nearby, you'll get a defense. While this is good, it doesn't count towards this achievement.
Class StruggleWork with a friendly Medic to kill an enemy Heavy & Medic pair.
This one can be infuriating or simple, depending on the skill of you and your Medic as well as the opposing pair of a players. The best way to do this is to ambush the pair so that you can eliminate the Medic before the Heavy can respond. You could probably do it in a head-on battle, but it would be much more difficult than a simple ambush would.
To farm this one, just have a Heavy & Medic pair stand still while you murder them. Make sure the Medic is healing the Heavy, or it doesn't count. A good achievement to do with this one is Photostroika. Just make sure your Medic übers you as you kill either member of the pair.
Tip: As mentioned above, the Medic must be healing the Heavy as you kill him, or it won't count.
Soviet BlockWhile invulnerable and on defense, block an invulnerable enemy Heavy's movement.
This one is interesting, to say the least. Basically, play a map like Goldrush where you can set up an über before the round starts. Chances are an enemy Heavy and Medic pair will do the same. When you see them über, do the same, rush forward, and block them from moving. Much like football, really (hence the icon).
To farm this, get an opposing Medic and Heavy pair to über and move forward. Then have your Medic über you and move forward, stepping in front of the enemy Heavy. As long as you block his forward movement, you'll get the achievement. Pretty easy, really.
Tip: You don't have to block all movement, just forward movement.

played a few long sessions this weekend (finally put down soul calibur 4), and thru just playing as heavy got milestone 2. though some of the chivos are totally farming oriented (20 kills with neither medic nor heavy dieing), most are pretty easy. ill just get to my 22 chivos n at that point if i get one i get one. unless those bastards at valve decided that getting 100% on chivos gets u some sorta perk, ill forever only have all medic chivos n bare minimum for the rest.
